Initial Evaluations

Our initial evaluation forms the foundation for our work together. This is a comprehensive, collaborative assessment where we'll explore your concerns, history, experiences, and hopes for treatment in depth. 

What to Expect: During the evaluation, I'll take time to understand your complete story, including your concerns, the context of your life, relationships, stressors, and strengths. We'll discuss what's been tried before, what's worked, and what hasn't. Together, we'll develop a personalized treatment plan that may include medication recommendations, therapeutic strategies, lifestyle approaches, or referrals to additional resources. 

For children and teens, parents/guardians participate in the full session to provide important developmental and family context. The extended time during the initial evaluation for children and teens allows for this collaboration.

Important Note: The initial evaluation is a one-time assessment fee. A treatment relationship begins only when we mutually agree to move forward together after the evaluation is complete. Ongoing care is billed separately at the follow-up session rates.

We’ll plan regular check-ins to monitor progress, address any concerns, and make adjustments to our treatment plan as needed. I will explain everything clearly so you (and your family) understand your options and why it will help. Whether adjusting medication, learning new skills, or navigating life changes, you'll have consistent support throughout your mental health journey.

Pricing

Standard Follow-Up (25 min): $300
Regular sessions to monitor progress, adjust treatment plans as needed, and provide ongoing support.

Comprehensive Follow-Up (50min): $500
Extended sessions ideal for addressing complex issues, building coping skills, or working through significant transitions.

Additional Services

Form Completion: $50-100
Insurance forms, school forms, camp forms, and others, if needed outside of a scheduled appointment. Fees based on complexity and time.

Letters or Extended Coordination of Care: $150/15 minutes
Includes requests for letters, document preparation, or extended coordination of care with other providers. Please discuss your needs during a scheduled appointment so we can plan accordingly. Fees based on complexity and time.

Out-of-Network Insurance: 

For patients with other insurance plans, Constellation Psychiatry is considered out-of-network. I provide detailed superbills that can be submitted for possible out-of-network reimbursement. Reimbursement varies by plan.

Find a time on the booking portal to request your first visit. You'll answer a few brief questions when booking to help ensure I'm the right fit for your needs. I’ll review and confirm with you within 1-2 business days.

Complete Intake Forms

Once your appointment is confirmed, you’ll receive access to the patient portal and intake forms to complete before our visit. The information you provide will help make sure we can make the best use of our time together.
